For Students Applying through the College Foundation of North Carolina (CFNC):
1. Counselor Statement
  For first-year applicants applying through CFNC only. Serves as a cover sheet for the applicant's official school transcript. To be completed by the applicant and the school counselor.
2. Counselor Supplement
  For first-year applicants applying through CFNC only. Schools who are submitting counselor statements for many students may use this fillable form to report school-specific information, and complete only the student-specific information on the regular Counselor Statement. To be completed by the school counselor.
3. Teacher Recommendation
  For first-year applicants applying through CFNC only. To be completed by the applicant and the teacher providing the recommendation.
For All Admitted Students: Enrollment Reply Form

For International Students:
1. 2013-2014 Financial Certificate
  Required of admitted international students only. Students needing I-20 documentation to receive an F-1 visa must submit this form prior to enrolling.
For Transfer Applicants:
1. Community Standards Form
  For enrolling transfer students only. Students who are enrolling for Fall 2013 should submit this form by May 15, 2013.

For Current and Former Students:
1. Transfer Credit Evaluation for Current Students
  For currently enrolled Carolina students who wish to have additional transfer credit applied toward their degree.
2. Readmission Application
  For students who were once enrolled at Carolina but withdrew or failed to complete a semester.
Other:
1. Part-Time Classroom Studies Application
  For part-time local adult students who seek to enroll in no more than eight credit hours each semester and don't already have a bachelor's degree.
2. Employee Undergraduate Degree-Completion Application
  Application for part-time undergraduate degree-completion program for UNC-Chapel Hill employees.
