First Year Application
Includes all parts of the freshman application, including the Counselor Statement, the Teacher Recommendation form, and the North Carolina Residency Short Form, all of which are available separately below.

For admitted transfer students only. Hard copy of community standards form which must be completed by an official at the last college attended and submitted to complete enrollment.

Counselor Statement
For freshman applicants only. Serves as a cover sheet for the applicant's official school transcript. To be completed by the applicant and the school counselor.

Teacher Recommendation
For freshman applicants only. To be completed by the applicant and the teacher providing the recommendation.

Transfer Application—Fall 2008
For candidates applying for transfer admission from another accredited college or university. It is for all UNC-Chapel Hill degree programs except junior nursing and second-degree nursing.

Transfer Nursing Online Application
You may apply online for junior or second-degree admission to the UNC-Chapel Hill School of Nursing. The online application is available at http://nursing.unc.edu/.

International Application
For students who are neither citizens nor permanent residents of the United States; includes the Financial Certificate Form, which is also available separately below.

Financial Certificate Form
For students who are neither citizens nor permanent residents of the United States.

Readmission Application
For students who were once enrolled at Carolina but withdrew or failed to complete a semester.

Part-time Classroom Studies (formerly Continuing Studies) Application
For part-time local adult students who seek to enroll in no more than eight credit hours each semester and don't already have a bachelor's degree.
